Perinatal Navigator - Job Overview

Schedule: Monday-Friday (8AM-4:30PM or 8:30AM-5PM) | Schedule Dependent on Site's Walk-In Testing Schedule

Key Responsibilities

  • Conduct point-of-care testing, health screenings, and risk assessments using cutting-edge technology.
  • Facilitate client appointments and follow-ups through advanced scheduling platforms, ensuring seamless patient experiences.
  • Implement innovative outreach strategies to connect with and support pregnant individuals effectively.
  • Deliver culturally sensitive education on prenatal care and healthy practices, driven by data insights.
  • Perform brief screenings of patients' behavioral, social, and medical needs, leveraging predictive analytics to guide readiness for health behavior change.
  • Support patients in setting and achieving transformative health goals.
  • Enhance retention strategies for OB and pediatric patients through creative postpartum follow-up and family engagement techniques.
  • Streamline access to community resources such as WIC and behavioral health services.
  • Meticulously document all patient interactions in Legacy's state-of-the-art electronic health record system.
  • Collaborate within an interdisciplinary clinic team to innovate and enhance care delivery.
  • Achieve performance metrics aligned with grant and contract requirements, driving continuous improvement.
  • Engage in ongoing learning and development through workshops and cross-clinic collaboration.

Minimum Qualifications

  • High School Diploma or equivalent, or a unique blend of relevant education, training, and experience.
  • 2-3 years of experience in a forward-thinking healthcare environment.
  • Proficiency with computers, including experience with advanced software programs.
  • Capability to provide point-of-care testing within three months, keeping pace with technological advancements.
  • Adaptability to rapidly develop core competencies in an evolving healthcare landscape.
  • Desired: Bilingual in English/Spanish to enhance patient engagement and communication.
  • Experience with electronic health record systems is highly advantageous.
  • Background working with OB and Pediatric populations is a plus.
  • Experience in engaging with sexual minorities and gender non-binary individuals.
  • Proven ability to educate about pregnancy and the postpartum period.
